These are material terms of the current West Virginia Lottery request. Read the official form yourself before signing; The Playbook does not receive or submit your information.
Scope is specific
CAS-F001 names the four West Virginia racetrack casinos, the Historic Greenbrier Casino and associated West Virginia-regulated iGaming and sports-betting mobile/online applications. It does not include iPLAY.
It does not expire after one year
The exclusion is permanent unless the West Virginia Lottery Commission or a court determines otherwise. After at least one year, you may request removal with CAS-F003; you remain excluded unless and until removal is approved.
List membership is a public record
Do not treat statewide self-exclusion as anonymous. The form authorizes necessary sharing with casinos, agents, affiliates and mobile-gaming services. This does not mean every supporting identity field is publicly displayed.
Entering or using covered access can have consequences
The form warns about removal from covered premises or services and possible trespass consequences. The participant remains responsible for avoiding covered access after enrollment.
The official form requests sensitive identity proof
The current request asks for identifying information and government photo ID; remote completion outside West Virginia Lottery Security personnel requires notarization. Submit only through the official instructions—never to The Playbook.
Effects outside West Virginia can differ
A casino or affiliated company may extend restrictions in another jurisdiction, but the checked material does not prove one universal cross-state rule. Confirm separately before relying on an out-of-state effect.

You are increasing stakes, redepositing or planning another bet mainly to win back money.
Do now: Stop the session, make no recovery wager and call 1-800-GAMBLER before reopening the account.
2 · Gambling is using credit or essential money
A wager would use borrowed funds or money needed for housing, food, utilities, transport, medicine or another committed bill.
Do now: Make no deposit. Move to a lawful shared-finance safety plan and ask the local helpline for financial-management resources.
3 · A time or money limit is being broken
You are extending the session, changing a plan under pressure or repeatedly overriding a limit you set.
Do now: Use the strongest available decrease, timeout or suspension now, then use the product map for a broader barrier.
4 · You are hiding the play or its cost
You are deleting records, minimizing losses, concealing accounts or avoiding a person who shares the financial impact.
Do now: Stop, preserve the records and tell one trusted person what support you want. Do not share passwords or one-time codes.
5 · Gambling has become a way to escape distress
You are gambling mainly to numb fear, anger, loneliness, grief or another difficult feeling.
Do now: Contact gambling help now. If the distress is a mental-health or suicide crisis, call or text 988; use 911 for immediate danger.
6 · You are trying to get around a barrier
You are opening another account, device or location after a timeout, blocker, closure or exclusion decision.
Do now: Add human support to the barrier: call 1-800-GAMBLER, use the correct formal route and ask about a Gamban activation code for an additional device block.

1-800-GAMBLER helps loved ones as well as the person gambling. Ask for help planning the conversation, protecting shared finances and finding local or telehealth support.
Do not impersonate or force-enroll
Do not enter another adult's account, submit their identity, sign a voluntary exclusion form for them or claim they consented. The current state program is voluntary and participant-submitted.
Protect shared money lawfully
Separate essential bills, review jointly held accounts and use alerts or access changes you are authorized to make. Do not move or freeze money owned solely by someone else without authority or individualized advice.
Offer one concrete next action
Ask whether they will call support with you, open the product map or read the official form consequences. A refusal does not prevent you from calling the local network for your own support.
